The Chaos Bauble (d100)
A fun cursed item which blatantly wants to kill you
From the original author u/WeaponizedOrigami:
This is a homebrew item that I've had tremendous fun with. The chaos bauble is a beautiful piece of blown glass on a fine silk cord, which can be worn like a bracelet, tied to a belt, or wrapped around the handle of a sword. When rubbed or polished it triggers a random occurrence determined by a d100 roll. There is no daily limit. If a spell is indicated the user may make a save to control the direction of the spell, and if the save is failed the spell may hit allies or structural elements (e.g. the player might accidentally blow a hole in the ceiling or fell a nearby tree.) The spell can be found on the indicated page of the fifth edition player's handbook. If a monster is indicated the monster will appear, but will not necessarily attack. The player may attempt to trap, charm, or reason with the monster, or attempt to incite it to attack a foe. The monster can be found on the page indicated, either in the 5th or 4th edition monster manual (4th edition monsters are listed as mm4.)
The chaos bauble does not like the player and can always be assumed to be acting with malice. But it's just so much fun!
